Previous experience in Home Care / Healthcare Intake will receive priority
Fluent in English (spoken and written) — mandatory
Moderate proficiency in Hindi, Urdu, or similar languages is an advantage
Strong communication and telephone-handling skills

Ability to manage multiple cases simultaneously
Reliable attendance and strong work discipline

HouseCalls Home Care is looking for a highly organized and responsible Intake & Admissions Coordinator to become the first point of contact for new clients and their families.

The successful candidate will manage the intake, Medicaid eligibility, UAI coordination, documentation, and client onboarding process, ensuring a smooth transition from referral to service delivery.

Receive and manage incoming referrals and client inquiries
Verify service area, care requirements, insurance, and Medicaid eligibility
Guide families through the Medicaid application process
Collect and organize required documentation
Coordinate UAI (Uniform Assessment Instrument) screenings
Follow up with DSS, Health Department teams, and MCO care coordinators
Track service authorization and resolve documentation gaps
Complete onboarding paperwork and service agreements
Maintain accurate client records in the company system
Coordinate with Clinical, Scheduling, and Billing teams
Maintain clear communication with clients and their families throughout the intake process
